Using the McGivney hemorrhoidal ligator

With this design, one hand grasps the hemorrhoid with a separate forceps. Meanwhile, the other hand advances the drum and fires the band. For that reason, most teams work with an assistant holding the anoscope.

Setting up the instrument

Before the procedure, load one or two bands onto the drum with a loading cone. Then check the trigger moves the pusher cleanly across the drum face. During banding, draw the tissue well into the drum, because a shallow bite can slip. Also, keep the anoscope still so the target stays in view. After each band, confirm it sits above the dentate line, where there are few pain fibres. Finally, reload before the next site rather than mid-procedure.

Care routine

Separate the parts after use where possible. Then clean, rinse and dry each piece. Finally, reassemble and test the firing action before you sterilize it.
