The right-angle shape of lower forceps

Down in the mandible, these beaks turn roughly ninety degrees off the handle line. Because of this, you can reach down onto the root while your hand stays above the teeth. It also lets you stand in front of or behind the patient, as you prefer.

How to narrow the choice

Start with what is left of the tooth. If the crown is sound, a standard figure is usually enough. However, if decay runs below the gum, a deep-gripping figure reaches further down the root. The fitting handle versions keep the same figure but give an anatomical grip. So pick them if you like a fuller handle in the palm.

Working on a lower premolar

First, free the gum and seat the beaks on root surface, not on enamel. Then drive them apically with steady pressure, since lower premolar extracting forceps need a deep hold. Next, use controlled buccal and lingual movement with a little rotation. Once the socket widens, deliver the tooth upward and out. In practice, the lower premolar root is often round, so rotation helps here.
