Hirschman Anoscope

The Hirschman anoscope is a familiar office instrument for looking at the anal canal. Three diameters come at 65 mm, and a longer 125 mm tube reaches further.

Picking a diameter

The 16 mm tube suits a tight or tender canal. Meanwhile, the 19 mm size covers most adults. For a wider field, choose 22 mm.

The 125 mm tube

The 22 x 125 mm version extends the view into the lower rectum. So it helps when a lesion sits beyond the reach of a 65 mm tube.

Office routine

Many clinics keep this anoscope on the procedure tray for every anorectal visit. First, lubricate the tube and obturator, then insert them together. Next, remove the obturator and inspect as you withdraw. Also, rotate the tube a little to bring each wall into view. Because the 16 mm size is gentler, keep it for tender patients or after recent surgery. Finally, return each size to its own slot so staff can find it quickly. For teaching clinics, the long 125 mm tube also gives trainees a clearer view of the lower rectum.

Care of the Hirschman anoscope

Rinse and brush the tube after every patient. Also, dry it fully before sterilization, because trapped water can mark the steel.
