Why straight

A straight shaft keeps the tip exactly in line with the hand. Therefore the surgeon can feel small changes in resistance as the plane opens. That feedback helps avoid tearing the lining.

In practice

First, the surgeon makes an incision near the front of the septum. Then the elevator starts the flap and follows the plane backward. Finally, the surgeon uses other tools to correct the deviation.
