Gustav Killian shaped much of early septal surgery, and the Killian gouge carries his name. This curved gouge removes bone inside the nose under gentle mallet taps.

Why curved

A curved gouge reaches bone at an angle. As a result, the surgeon can work around the nasal anatomy instead of against it. It also offers a better view past the hand.

Septoplasty sometimes involves bone at the base of the septum. For instance, a crest may need lowering before the septum can straighten. Then the surgeon uses a gouge to take that bone down.
