De-Martel Wire Saw Guide
The De-Martel wire saw guide helps a surgeon pass a wire saw under the skull. It runs between two burr holes, and then the saw cuts the bone above it while the guide shields the dura.
Why a guide matters
A wire saw cuts on the pull. Because the saw sits against the inner skull, the dura below needs protection. The guide lies between the bone and the dura, so the wire runs on top of it instead.

Steps with a wire saw
First, the surgeon makes two burr holes. Next, a dissector frees the dura between them. Then the guide passes from one hole to the other, and the wire saw follows along it. Finally, the surgeon pulls the saw back and forth to cut.
Completing the set
This guide works with a wire saw and a pair of handles. In addition, a dura dissector should sit on the same tray. Many teams keep a spare guide, since it is small and easy to misplace.

Rinse the guide right after the case to clear blood and bone dust. Then clean any channel or groove with a thin brush. Dry it thoroughly and inspect it for bends before sterilizing.
