The Brunner sternum scissors divides the sternum in the midline. It works on compound leverage, so the blades close with far more force than the hand applies. This model measures 32 cm. Also, it is built in two mirrored versions, one for each operating hand.

Why the Brunner sternum scissors is handed

Sternal bone is dense. The cut therefore needs both power and a straight line. The instrument is offset so the blades sit in the midline while the handles clear the chest wall. As a result, the operator can drive the cut without twisting the wrist.

That offset only works in one direction. In other words, a right-hand instrument suits a right-handed operator. Used in the other hand, the blades load unevenly and the line of the cut wanders. So the two versions are mirror images rather than a stocking convenience.

Choosing right or left

Match the version to the operating hand, not to the side of the table. For example, a left-handed surgeon takes the left-hand instrument even when standing on the patient’s right. Otherwise the two are identical in length, blade pattern and finish.

Care

Bone is hard on an edge. So inspect the blades after every case and look for chips along the cutting line. Bone dust also packs into the joint, and a joint that is not cleaned out will stiffen and then bind under load.

Dismantle or open the joint fully for cleaning. Then dry it completely and lubricate before storage. Finally, close the blades slowly by hand and check that they meet along their whole length.
