Description
Coakley Trocar
The Coakley trocar lets ENT surgeons puncture the maxillary sinus through the nose. Once inside, the stylet comes out and the 2.25 mm cannula stays put, so the sinus can be washed out with saline.
Antral puncture
The tip passes beneath the inferior turbinate and through the thin bony wall of the sinus. Consequently, trapped mucus or pus can drain without an external cut.
Size of the Coakley trocar
A 2.25 mm channel is wide enough for washout and sampling. Even so, the small opening in the sinus wall closes on its own afterwards.
When ENT teams use it
Antral washout helps in stubborn maxillary sinusitis. In addition, the fluid that returns can go for culture to guide treatment.
Preparing the nose
Before puncture, the clinician numbs and shrinks the nasal lining with topical medication. After that, the trocar meets far less resistance and the patient feels little.
Washing out
With the stylet removed, a syringe of warm saline attaches to the cannula. Gentle flushing then carries the contents out through the natural opening of the sinus.
Checking the point
A crisp point passes the bony wall cleanly. For that reason, inspect it before each list and replace a worn stylet promptly.
